The Medical Statement Form
Your scuba course may begin as soon as you sign up for the course of your choice. However, before you may do any confined water training, you need to have a completed Medical Statement form on file. This form is available in pdf format. You will need Adobe reader installed on your PC to download this form. To get Adobe reader click on the "Get ADOBE READER" icon below. How to Properly Fill Out the Medical Statement Form
The form lists a number of medical conditions that may require your physicians approval for fitness to dive. On the form, next to each potential medical condition write the word "yes or "no" as applicable. Checkmarks or other notations can not be accepted. If you have completed the Medical History portion of the form with all "no" answers, fill out the Student portion on the reverse side, sign the form where indicated and bring it to MidCoast Scuba Center. That is all you need to do regarding the Medical Statement form.
Should you have one or more "yes" answers to the listed medical conditions, sign the form, fill out the reverse side, have your physician check the appropriate box as to your fitness to dive and sign the form. We must have this form fully and properly filled out before you may participate in any confined water activities.
Physician's Guidelines
